Sounds like a Mac specific problem, but if it isn't, check computer folder permissions and check if admin.

Its not a Minecraft issue, your internet is bad. Another possible cause is the server host has a different ip than you are attempting to connect to.

Not sure, but looks like a mod called advancements is crashing it.

if that doesn't work you need to uninstall and reinstall minecraft.
First backup the world. Than try loading the world onto a special launcher because that seems like the only doable option.
