Are All users Here using JEI? ether its a JEI loading issue or minecraft can't handle loading the texture. its 3D and it says in the crash report it can't load the texture in your inventory. you might be loading in the M4A1 because i have figured out the M4A1 dose not load properly in game with the /Give command. try using optifine but optiforge is buggy.
